Our city-wide service provides therapeutic assessment, management and treatment for people over the age of 18, registered with a Newcastle GP.
People that are most appropriately treated in their own home are referred to the service, due to their problems being intrinsically linked with the home environment and travel to and from the hospital negating any benefits obtained.
Main conditions referred to our team
The majority of our patients are elderly and many have multiple, complex conditions requiring an holistic/eclectic approach.
The main conditions we see are related to:
- elderly mobility and falls
- musculoskeletal conditions
- respiratory conditions including COPD
- neurological conditions
- palliative care
Range of services provided
Our broad range of clinical skills allows us to offer home exercise programmes, equipment provision, acupuncture and manual therapy in order to maximise independence in the home.
We also have close links to other associated services that sometimes involves collaborative working, or, may require a referral to another team.
