Community stroke teams
Your community stroke team usually includes several different health professionals who all have specialist training and experience in stroke.
Their role is to help support your recovery and rehabilitation (recovering and adjusting to life) in the first few weeks and months after you leave hospital. This includes helping you with adjustment and living well after your stroke. They can also help and support families and carers.
Your team may include
- Community stroke nurses

- Speech and language therapists
- Occupational therapists
- Social workers
- Dietitians
- Psychologists
- Rehabilitation assistants
A member of the team will contact you following your discharge from hospital. The team will know about your stroke and the therapy you have been doing in hospital. The team will plan with you how to carry on with your rehabilitation.
Together, and with the help of your relatives or carers, the team will work on the things that are important to help you get on with your life again.
Everyone’s needs are different after a stroke.
