- 3 or more relatives diagnosed at any age
- A family history of a known hereditary colorectal cancer syndrome eg. Lynch syndrome (HNPCC)*, Familial Adenomatous Polyposis (FAP), Peutz-Jeghers syndrome (PJS), MYH-associated Polyposis
