Primary school aged health assessments
You have open access to the health visiting service (Mon-Friday 8.30am-4pm) by calling the Single Point of Access 0191 282 3319 if you need support or guidance.
All children and families are entitled to a universal service. The level of support offered is assessed on each individual families health, development and social need.
We send out pre-school questionnaire letters to parents for children before a child starts primary school and again in Year 6. These help parents, carers or children identify anything that may impact on a child’s ability to achieve their potential.
Pre-school assessment can help to support a child’s smooth transition into primary or secondary school. We can provide advice, guidance and support for a range of issues such as behaviour, healthy lifestyles, growing up, puberty, toileting and emotional wellbeing.
