These health assessments look at the young person’s physical, emotional and social needs. These health assessments can identify any advice, support or help that may be needed to prepare the young person for adulthood.
These can be held in clinics, school or your home. We make sure that we listen to your concerns and that your voice is heard in these assessment. This is chance for you to talk through any worries, concerns or questions you may have.
Through these assessments school nurses gain an understanding of any existing health needs you may have and can signpost, or refer you into other services or offer individual support.
This can include a range of health issues such as:
- Advice with relationships (eg sexual health, healthy relationship concerns or bullying)
- Identity exploration (sexual or gender)
- Emotional health and wellbeing (eg exam pressures, bereavement)
- General health concerns
This support can give young people strategies to build resilience and to manage difficult situations and stress for future.
