A key priority for our NMAHP Strategy is ensuring our staff have the autonomy to influence decisions on how care is structured and delivered, harnessing their expert knowledge and skills to lead improvement and reduce patient harms whilst providing the highest quality, compassionate care to our patients in our hospitals, outreach clinics and in the community.
Our Trust has an expert team of community matrons – highly skilled practitioners who offer intensive care and support for patients with many long-term conditions, supported by two staff nurses and a healthcare assistant.
The matrons have the expertise to identify patients with the highest level of need and planning interventions with the aim of improving their quality of life and health outcomes, as well as reducing unnecessary hospital admissions.
They work closely with GP and social care colleagues alongside other members of the multidisciplinary team and the patients themselves with their family and/or carers to manage their health conditions through bespoke care plans, devised to meet their personal needs.
