The Patient Safety Incident Response Framework (PSIRF) represents a significant shift in the way the NHS responds to patient safety incidents and is a major step towards establishing a national safety management system.
The framework provides a coordinated response to analysing patient safety incidents, using data and insight. It ensures that the response to incidents is clearly linked with appropriate improvements in a measurable way.
The focus is all about reducing risk or harm, and is underpinned by principles of accountability, learning and continuous improvement.
Using the framework we have reviewed our quality and safety data over a three-year period and joined this up to fully understand where we need to focus our improvement work, so patients receive safer and more effective care and treatment.
This data review has also allowed us to identify the approach we will take to continually learn and make improvements to the care we provide.
